4.  Change the Group name from R1 to Q1_P_1 and the Value from V2 to 2. 

5.   Repeating Steps 1 4 in defining database fields for the rest of the buttons in the 

table. Make sure to change the Values and Group Name accordingly. 

After you have re defined the Group Names and Values for all the buttons, we will need to 

create these creates for the database.  To create data fields in the database: 

1.  Right click anywhere inside FDA
