Designing Your First Form 

What Is A Form? 

It is a structured way of collecting information through the use of a survey or census. Forms 

used to collect attitudinal information are called surveys. Forms used to collection any other 

type of feedback, personal data, transactional data, are simply called forms. If data is 

collected via the internet, the form used for this purpose is called an online form. Online 

form technology is still evolving. It still takes more time to develop, but the future promises 

software packages that will provide quicker design and deployment. After all, if technology is 

not making us smarter, it is making us do things faster. 

Below is a sample of an online form.  This was an actual online survey conducted by the 

Overseas Chinese Association of Institutional Research (OCAIR) in 2001. We consider this 

a good model that utilizes most of the features that you need to master. We will learn how to 

create this form in FP 2000 in the next few hours.
